Directions for completing the Medical Record Release Form

• Download the Medical Records Release
• Fill out the form completely, sign and date authorization.
• Include the name, phone number and address of where the records need to be sent.
• Include the visit dates or date range that is needed
• Include the appropriate Facility Name and Address of where you were seen (see the list below)
• Email the release to [email protected] or Fax to 828-253-1123

*Please note this release is for Asheville Orthopaedic Associates Office Notes only.  Any notes from Physical Therapy, Surgery or Radiology Reports will need to be sent directly to that facility where you were seen.
